GP Practice Service.
Kate Keays and Chris Smith share this role and are now making contact and visiting all of the GP Practices in Regis, Chichester and Rural, as well as other healthcare professionals working in the community.
Thier role is to support GP Practices in identifying carers and encouraging them to be recorded on the carers register within thier own practice. Additionally, it is hoped that a link worker will be appointed in each GP Practice who can be available to support carers in any way possible.
This will include advising carers about the services available through the Carers Support Service and encouraging them to sign up to recieve regular newsletters and information, keeping them informed of opportunities available to them.
Kate and Chris are happy to provide Carers' Awareness Training to healthcare professionals and would welcome the opportunity to meet with the Patient Participation Groups.
